The Mental Health Intensive Case Management (MHICM) Rural Access Network for Growth Enhancement (RANGE) Registered Nurse will be a member of the MHICM-RANGE Team and will provide case management services to veteran patients in the Dallas County area and will operate out of the newly established VA Outreach Center in Selma, Alabama.  The MHICM-RANGE team will expand access of mental health services to veterans residing in rural Dallas and surrounding counties in Alabama.  The team will work collaboratively with the existing MHICM Team at the Tuscaloosa VA Medical Center, who will serve as the host team.  The MHICM RANGE RN is responsible for the coordination of care for veterans within the program.  The target patient population will include those veterans who reside within a practical driving distance from the office in Selma, Alabama; have a high use of VA psychiatric inpatient resources or who have severe functional impairment or are inadequately served by conventional treatments, as well as veterans who are frequent users of emergency or crisis services; and who have a diagnosis of serious mental illness with or without secondary substance abuse.  The MHICM-RANGE RN will perform medication/illness monitoring and education; crisis intervention; serves as a patient advocate, outreaching to various community agencies to support MHICM veterans to experience successful community placement.  The MHICM Nurse demonstrates leadership and initiative in delivering and improving holistic care through collaborative strategies with others, ensuring compliance with the mission, vision and values of the medical center and/or VISN and is accountable for the delivery of quality, age-specific healthcare; for utilizing critical thinking skills and sound clinical judgment to ensure positive patient outcomes; for demonstrating ethical practice; and for participating in performance improvement activities to improve the quality of care and ensure performance measures are met and/or exceeded.  The incumbent supports the medical center's mission by possessing and exemplifying the Eight Core Competencies of the High Performance Development Model (Interpersonal Effectiveness, Customer Service, Systems Thinking, Flexibility/Adaptability, Creative Thinking, Organizational Stewardship, Personal Mastery and Technical Skills).
