Job Description

upervise the direct support staff, provide and coordinate care for residents, ensure that the assigned group home function according to established practices, policies and procedures, serves as the primary point of contact for parents/guardians and support coordinators.


Essential Job Functions

• Supervise the work activities of Direct Support Professional staff including communicating information, evaluating performance, and training/coaching.

• Effectively communicate and support the organization’s mission, vision and values.

• Work with the people we support and other members of the interdisciplinary team to develop service plans that aid the person in achieving their personal life goals.

• Assist the people we support to overcome barriers and challenges to establishing and maintaining a network of relationships, valued social roles and teach them new life skills.

• Accurately document services that are provided to include: daily progress notes, appointment summaries, medication administration, behavior and incident, billing and other documentation as required.

• Provide positive behavior support to the people we support as necessary and document accordingly.

• Work in partnership with the people we support to promote their health and safety including scheduling and transporting individuals to medical, dental, and other appointments.

• Promote and support community involvement for the people we support.

• Always communicate in a respectful manner with all people including the people we support, supervisors, other valued team members, and members of the community.

• Complete all required trainings and demonstrate competency through application of these trainings in daily job duties. • Maintain the confidentiality of the individuals that we support and that of the company.

• Maintain the assigned facility, order groceries/supplies, and participate in the on-call rotation.

• Other duties as assigned.

Minimum Job Qualifications

• Must have previous supervisory or leadership experience.

• Must have a passion for and desire to work with people with intellectual disabilities and other physical or mental illness. 

• Must have sufficient computer skills and excellent writing skills to prepare documentation, staff schedules, monthly activities calendar, compile data, etc. •

Ability to prioritize conflicting deadlines.

• Ability to observe, record and report participant’s behavior, attitude and physical condition.

• Ability to perform basic math and communicate effectively, as well as excellent verbal and written communication skills.

• Ability to effectively train program staff and provide on-going supports to encourage professional development of all staff

. • No convictions for crimes that preclude a person from working with members of a vulnerable population.

• Bachelor’s degree preferred.

• Valid driver’s license.

Physical Demands

• Ability to lift at least 50 pounds or more.

• Daily standing, bending and lift.

• Must be able to stand for at least eight hours at a time.
