Develops, monitoring, implements, & revising in collaboration with client(s), an individualized person centered treatment plan base on the assessment including: patients problems, specific and measurable short and long range goals and specify the treatment intervention with a multidisciplinary treatment team. . 5. Performs voluntary and court ordered assessments, including related diagnostics and testing, for clients needing substance abuse treatment and other services; 6. Provide on-call after hour”¦s consultation on a rotating schedule. 7. Dictates discharge summary when client completes treatment and has it entered in the medical record within one week after discharge. The summary is to include the reasons for discharge, status at discharge, final evaluation of progress, aftercare recommendations and required signatures. 8. Make contacts with new or potential referral sources in the community for services we provide. 9. Enters progress notes into the patient”¦s record documenting the content of contact with the patient or with collateral sources for the patient. Progress notes shall be filed in the medical record of the client within 24-48 hrs and include patient progress as related to the treatment plan and be signed and dated producing a continuous and unbroken record; 10. Completes Medicaid and other insurance forms when indicated in a timely manner. 11. Documents transfer or change made in the patient”¦s level of care and includes the basis for the recommendation using approved placement criteria, date of transfer and name of the facility to which the client is transferred. 12. Multiple contacts with consumers may be as frequent as 2 or 3 times per day, seven days per week and depend on individual need and a mutually agreed upon plan between individuals and program staff. Many, if not all staff shall share responsibility for addressing the needs or all individuals requiring frequent contact. An average of 3 contacts per week for all consumers. 13. 80% of services must be face-to-face with consumer & 75% of services contacts must be in the community in a non-office based or non-facility based setting. 14. Participates in daily staff meetings & supervisory meetings with QP. 15. Assurance of Client Confidentiality, Rights, and Safety. 16. Knowledge of agency Policies & Procedures. 17. Keep all trainings required by the State and local agencies. 18. Perform other duties as assigned by Administrative Team. 19. Keith Alcohol and Drug Abuse Treatment Center (JFK ADATC), is an 80 bed residential and detox/crisis stabilization treatment facility located just outside of Asheville in Black Mountain, North Carolina. We are designed to meet the needs of alcohol and other drug dependent adults of the 38 westernmost counties in North Carolina. We are currently recruiting for a Substance Abuse Counselor to serve as a case manager for developing, implementing, and coordinating therapeutic treatment.
