Position: Behavior Coach
Unique Family Services, LLC is currently seeking Behavior Coach to join its outstanding team.
General Description of Duties
The behavior coach is responsible for assisting individuals to eliminate or replace disruptive, harmful or negative behaviors with positive actions. This position plays a key role in the effective implementation of the program’s philosophy and program.
Role And Responsibilities
The behavior coach works under the direction of the program supervisor and clinical director to help train youth 5 through 17 years of age and their families to implement and sustain effective interventions in the context of their day to day lives. This includes Maintaining the integrity of the program structure by:
Conducting initial intake, assessment and development of service plan.
Providing continual in-home or community direct services for youth with complex behaviors.
Providing school-based skill development.
Identifying youth and family strengths and needs to enhance the Child and Family Team (CFT) planning process.
Actively participating in youth and family treatment planning and CFT’s.
Teaching youth and family a variety of skills (living, social, self-care, household management, etc).
Applying appropriate physical intervention (Safe and Positive Approaches) techniques to manage resistance for clients presenting as a danger to self or others.
Identifying, connecting, and integrating families in to variety of community resources and outlets that build off individual and family strengths.
Creating and implementing behavior modification plans to teach new methods in dealing with challenging behavior.
Meeting all required deadlines for submitting documentation.
Maintaining organized case load.
Able to accept additional duties as assigned.
Must be at least 21 years of age
Must be CPR and First Aid certified.
Must have a level one fingerprint clearance background check
Must possess a valid Arizona driver’s license and a good driving record for the past three (3) years.
Associates Degree preferred
High School Diploma/GED required
Working Knowledge of case management, behavioral health preferred
Experience working with autism, aggressive behaviors, or coaching youth preferred
Candidates must be able to provide their own transportation to clients’ homes and the office.
Working Knowledge of Microsoft Office (Word, Excel) required
Knowledge, Abilities, and Skills
Behavior Coach must be task oriented and be able to handle multiple assignments at a time.
Behavior Coach must also have the willingness and ability to work under times of extreme stress in a highly charged human environment.
Understanding the implementation of Unique Family Services, LLC mission values and code of ethics.
Varied, Fee per Service
PT- 5 to 6 cases
Salary: $15.00 To $16.00
