JOB SUMMARY: Provides leadership to the health care team in the provision of psychosocial support services to patients and families.

ESSENTIAL FUNCTIONS:

• Provides Clinical Social Work consultation, counseling, and follow up services according to psychosocial assessment and service plan.
• In conjunction with the health care team and other in the community, pro-actively identifies and ensures that all identified services and resources are utilized in a timely and efficient manner.
• Demonstrates understanding and practice of the values, standards, and beliefs of the social work practice.

REQUIREMENTS: Three Five years ; Education: Master of Social Work License Required Certification: Current Maryland LCSW, LCPC or Psychology license Required.
