JOB REQUIREMENTS: POSITION SUMMARY: This position provides professional
mental health services and case management to children ranging from 5-18
years of age and their families; providing therapeutic intervention to
promote development and recovery within a natural environment. ESSENTIAL
FUNCTIONS: The following duties are normal for this position. These are
not to be construed as exclusive or all-inclusive. To perform this job
successfully, an individual must be able to perform each duty
satisfactorily. Other duties may be required and assigned. -Provides
mental health therapy/treatment including individual, family and group
treatment. This would include face to face, phone contact, and written
correspondence. -Conducts mental health evaluations and assessments;
including gathering information regarding symptoms, current and previous
psychiatric medications, previous treatment or therapy, history, etc.
-Formulates diagnosis; develops treatment options with client; makes
appropriate referrals to other providers and appropriate resources
within and outside of St. Croix County DHHS. -Documents assessments,
treatment plans, reviews, referrals and discharge planning and summaries
in accordance with professional standards, agency, state and federal
guidelines. -Bills for all allowable services and submit billing as
directed. Ensures state reporting system forms and data collection are
completed on a timely basis. -Performs ongoing assessment of client for
risk of harm to self or others, including developing crisis plans and
determining level of care when necessary. -Serves as a member of the
ACCESS team including initial screening, emergency services, crisis
stabilization, referral services, and care planning in consultation with
interdisciplinary team/s. -Provides consultation, collaboration and/or
training, screening services, psychoeducational and group facilitation
to internal agency divisions and community. -Serves as a mental health
professional on the Comprehensive Community Services and other
interdisciplinary teams including ACCESS, Behavioral Health teams, CCS,
CST, CFS. -Other duties as assigned. \*\*\*\*\* OTHER EXPERIENCE AND
QUALIFICATIONS: MINIMUM QUALIFICATIONS: EDUCATION AND/OR EXPERIENCE
REQUIREMENTS -Master\'s degree in social work, clinical psychology, or
related program. -Three years\' experience working in a clinic setting
with clients. -Experience working with young children preferred. -State
of Wisconsin Licensed Clinical Social Worker, Licensed Marriage and
Family Therapist, or Licensed Professional Counselor preferred. -State
of Wisconsin Advanced Practice Social Worker, Marriage and Family
Therapist, or Licensed Professional Counselor-In Training will be
considered. License is required within 1 year of employment. -SUDS
license and/or demonstrated education or experience working with clients
affected by SUDS, preferred. -Dialectical Behavioral Therapy (DBT)
training and experience, preferred. -Must successfully pass criminal and
