Mental Health Case Manager - Children & Families

Pathway Caring for Children is a nonprofit mental health agency dedicated to strengthening the lives of children and families for over 50 years. Through trauma-informed care, we offer support in foster care, adoption, and mental health services across Northeast Ohio.

We're hiring a Mental Health Case Manager to support children, teens, and their families with emotional, behavioral, and social challenges. This community-based role involves working in homes, schools, and neighborhood settings to deliver mental health support and connect families to vital resources.

Key Responsibilities:

• Provide individualized support in homes, schools, and community settings

• Collaborate with therapists to implement client-centered treatment plans

• Facilitate group sessions on social skills, conflict resolution, and coping strategies

• Help clients and families access community services and support systems

• Advocate for families in school, medical, and social service systems

• Maintain accurate case notes and documentation for compliance and quality assurance

Qualifications:

• High school diploma or GED required

• Experience working with children, teens, or families preferred

• Strong communication, organization, and problem-solving skills

• Ability to work independently while managing multiple priorities

• Must be 21+ years old

• Valid driver's license, reliable transportation, and current auto insurance

• Ability to pass required background checks

Schedule & Work Environment:

• Flexible weekday schedule with occasional evenings to meet client needs

• Work in various locations (homes, schools, community sites, and office)
