LifeWorks NW's Zenith program provides mental health residential treatment services for youth and young adult clients (17 to 25 years of age) who are experiencing severe and persistent mental illness and behavioral problems.

The Mental Health Therapist provides service coordination and transitional case management for individuals who reside atZenith RTH. Clinician will collaborate with community providers and assist withengagement and connection to services. This position is not responsible for fiscal aspects of the program.

Location: This position is based out of LifeWorks NW's Zenith Residential in Tigard, OR.

Pay/Benefits : $30.52 starting hourly pay plus, pro-rated comprehensive benefits .

EssentialResponsibilities:

  • Providesdirect service including facilitation of group and individual sessions, clientevaluations and case management.
  • Collaboratesand coordinates with other community providers to help individuals engage inservices.
  • Coordinates client incoming and annual updatesfor Comagine Health Care assessments for billing purposes.
  • Respondsto emergency situations involving residents experiencing mental health crisis.
  • Providesprogram mental health assessments and crisis intervention as appropriate.
  • Performsclient evaluations and risk assessments. Formulates a diagnosis and develops a short-termstabilization or safety plan to relieve the present mental health crisis and toprovide a stable environment for clients.
  • Prepares mental health evaluations and reportsfor client caseload. Maintains clear and concise records. Maintains case files that include activities,progress notes, evaluations, and recommendations according to confidentialityguidelines. Completes documentation in an electronic healthrecords system in a timely basis.
  • Providesfollow up contact as developed in the stabilization or safety plan. Provideson-going evaluation of safety plan and updates safety plan as needed.
  • Providesconsultation and training to staff and families regarding mental health issues,and behavior management as it relates to clients.
  • Maintainsawareness of community referral resources. Helps to build positiverelationships with other community providers and provides appropriate referralservices.
  • Assistsstaff members by helping to resolve difficult emergency situations and servesas back-up for other staff when needed.
  • Alwaysmaintains professional presentation and responds the program all situations /calls with a customer service attitude..
  • Travelsbetween sites and in the community, May transports clients in and/or personal vehicles.
  • Adhereto an On-Call rotation as a program administrator.

Qualifications:

  • Strong assessment, decision-making andinterpersonal skills are absolute necessities for this position.
  • Master'sdegree in Social Work, Psychology, Counseling, or a Mental Health relatedfield.
  • Valid and active QMHP credential certificationthrough MHACBO or a valid and active status (licensed or registered intern)with an approved Oregon licensing board.
  • Demonstratedability to conduct an mental health crisis assessment, including identifyingprecipitating events, gathering histories of mental and physical health,alcohol and other drug use, past mental health services and criminal justicecontacts, assessing family, social and working relationships and conducting amental status examination; complete a DSM diagnosis; work with the individualto create a safety plan and provide appropriate referrals to address chemicaldependency and/or mental health concerns.
  • Morethan 1 year of experience preferred in a broad range of clinical interventionsand mental health assessments with children and families or individual withsevere and persistent mental illness.
  • Demonstratedability to work effectively on a highly interdependent team. Necessary skillsets include initiative, dependability, flexibility, and strong organizationalskills.
  • Demonstratedability to maintain case management in accordance with federal, state and/orfunding source requirements as well as LifeWorks NW policies and procedures.
  • Knowledgeof community resources and treatment agencies enabling appropriate referralsfor specific client needs.
  • Knowledgeof medications commonly used by client population.
  • Goodcomputer skills including a functional knowledge of Microsoft Outlook and Word.Ability to master proprietary software including electronic health recordsystem. This includes the ability to enter data and retrieve data and toprepare simple reports.
  • Demonstrateseffective communication skills and maintains a cooperative working relationshipwith others in a culturally diverse environment.
  • Requires valid driver's license and verification of safedriving record from DMV.
  • Ability to work in the community and in person with clientsand community partners.
