Sunbelt Staffing, in partnership with a leading insurance provider , is seeking an experienced Nurse Practitioner (NP) to join an Institutional Special Needs Plan (ISNP) program serving four facilities across Morris and Union Counties, NJ . This position offers the opportunity to provide on-site, patient-centered care to residents in skilled nursing and assisted living facilities , focusing on reducing preventable hospitalizations and ensuring smooth care transitions.

Contract Details:
  • Employer: Sunbelt Staffing (on behalf of an insurance provider)
  • Type: 6-month contract
  • Schedule: M-F, 8 am- 5 pm (40 hours/week)
  • Locations: 4 facilities throughout Morris & Union Counties
  • Setting: Skilled Nursing Facilities (SNF), Assisted Living Facilities (ALF), Independent Living Communities (ILC)
  • Benefits: Medical, dental, vision insurance (effective first of the month after start date), 401 (k) after 90 days, no paid time off
  • Compensation: Rate negotiated with client based on experience and area of need
Position Overview-ISNP Program:

The Institutional Special Needs Plan provides comprehensive care for insurance members in nursing home and assisted living settings, with a focus on:

  • Treating patients in place whenever possible
  • Avoiding preventable ER visits and hospitalizations
  • Supporting smooth transitions back to home settings
  • Monthly (or more frequent) health assessments to identify care gaps and coordinate with primary care providers
Job Duties:
  • Complete comprehensive clinical assessments for each resident
  • Conduct advanced illness and disease trajectory discussions to establish personalized goals of care
  • Develop problem lists and medical treatment plans in collaboration with the residents PCP
  • Provide primary, preventive, urgent, and acute care onsite
  • Identify and stratify high-risk patients for increased visit frequency
  • Create contingency plans for high-risk conditions
  • Communicate with residents responsible parties and facility staff (DON, PCP)
  • Prescribe or refill non-narcotic medications upon PCP consultation
  • Coordinate care with the support of an RN Care Coordinator
  • Conduct single-visit assessments for newly admitted patients as part of post-hospital discharge planning (~23 per day, ~90 minutes each)
  • Travel between facilities as needed; typically scheduled at one facility per day, with flexibility for urgent needs
Requirements:
  • Minimum of 1 year of experience
  • Active NP license in New Jersey (or ability to obtain before start)
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification
  • Active BLS (AHA or ARC) certification
  • Strong background in long-term care, home health, or both
  • Experience with care planning, patient education, and interdisciplinary collaboration
  • Ability to travel between facilities within assigned counties
  • COVID-19 vaccination required
