Bilingual Psychiatric Case Manager (MHP)

SUMMARY
Provides case management for clients receiving psychiatric services. Responsible for coordinating with the psychiatric provider and providing support to clients during in-person and telehealth appointments, including interpretation for families when needed. Assists with client evaluation, treatment planning, and ongoing monitoring of progress. Coordinates care for individuals with mental health diagnoses and facilitates referrals, testing, and access to community resources to support continuity of care.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Clinical/Care Coordination
  • Coordinates psychiatric services and supports clients during in-person and telehealth appointments.
  • Orients new clients to psychiatric services and assists with required forms and telehealth setup.
  • Completes medication prior authorizations as needed.
  • Assists with GeneSight testing including consent, sample collection, and tracking results.
  • Acts as liaison between clients, families, and psychiatric provider.
  • Completes IM+CANS assessments and treatment plans.
  • Coordinates followup appointments and referrals.
  • Obtains records, labs, and outside reports.
  • Facilitates neuropsychological and psychological testing referrals.
  • Coordinates rating scales and screening tools.
  • Provides case management support and resource linkage.
  • Responds to client calls and crisis concerns.
  • Supports school documentation requests.
  • Maintains documentation per Medicaid Rule 140.
Supervision
  • Responsible for being prepared to discuss cases with supervisor in clinical supervision.
  • Reports to supervisor as needed
Administrative
  • Responsible for completion of and meeting deadlines for all clinical paperwork
  • Responsible for completion of and meeting deadlines for all agency administrative paperwork
  • Review notes completed by the provider for assigned clients
  • Responsible for attending clinical staff/program meetings, including Utilization Review.
  • Responsible for maintaining clinical charts in compliance with Medicaid Rule 140.
Community Education / Public Relations
  • Networking and outreach as assigned
Training
  • Responsible for fulfilling agency requirements for training and documentation
  • Responsible for attending all clinical agency in-service training.
  • Responsible for maintaining certification in IM+CANS
Special Assignments / Projects
  • Additional duties as assigned

POSITION TYPE/EXPECTED HOURS OF WORK
This position is full-time, which entails a 37.5 work week. Two evenings per week are required.
Hourly Pay Rates
  • In accordance with Illinois state law, we are committed to transparency in compensation. The hourly pay range for this position is $22.07 to $28.84 per hour, depending on experience and qualifications

TRAVEL
This position may require light travel.
EDUCATION AND EXPERIENCE
  • Must have bachelors degree in social work, counseling, psychology or similar field.
  • Bilingual in Spanish required.
  • Must be flexible, independent and able to multi-task effectively.
  • Preferred experience in working with youth with mental health issues.
