Pay is commensurate with experience within a range of $20.00 to $27.86 per hour. Full time, non-exempt, benefits eligible. Responsibilities: The primaryresponsibility of the Supportive Housing Case Manager is to work withpeople (mainly adults) whom are currently struggling with symptoms of mentalillness, that also need assistance with obtaining and maintaining independentcommunity housing. The consumers areserved by the Bert Nash Community Mental Health Center. Duties include but not limited to:
- To help clients to understand the symptoms oftheir mental illness and to minimize the effect of those symptoms in all thesignificant areas of their lives;
- Completestrength assessments, collaborate in treatment planning and review thetreatment plan with the client;
- Assistclients in learning and practicing news skills that contribute to theirrecovery as specified in their individualized treatment plan;
- Providesupportive counseling, solutions focused interventions; behavioral analysis toincrease client's ability to function in all life domains;
- Completeindividualized assessments with clients and identify strategies or treatmentoptions associated with the consumer's mental illness with the goal ofminimizing the negative effects of mental illness symptoms or associatedenvironmental stressors;
- Providingoutreach and crisis intervention services to clients and prospective clients toenable timely access to those services appropriate and necessary for crisisstabilization with the purpose of diverting the client from hospitalization andallowing the client to maintain independent living;
- Additionalresponsibilities that may be assigned as circumstances warrant;
- Flexibilityto provide some after hour assistance clients in accessing the differentlocations within the Bert Nash CMHC network of services;
- Regularand predictable attendance is an essential function of the job.
Qualifications : The Supportive Housing Case Manager positionrequires but not limited to:
- BA/BS degree (psychology,social work, or psychiatric nursing preferred) and/or four years of equivalent experience working in the humanservices field;
- Ability to relate and effectively provide services topeople who are experiencing mental health and housing related crisis;
- Experience working with individuals who are experiencinga serious and persistent mental illness, have a history of substance use andmight have experienced significant trauma;
- Must be able to lift 25 lbs or more to assist theclient in ADLs, IADLs;
- Must have reliable transportation and valid,unrestricted driver's license;
- Commitment in providing services with a TraumaInformed and Person-Centered approach;
- Also brings an energetic and positive approach to therapidly evolving changes within the housing program and the challenges of acomplex workplace;
- Flexibility in assigned working hours.
