What are we looking for in a Care Coordinator - WISe?
Care Coordinators are part of the WISe (Wraparound with Intensive Services) program, which serves youth who have mental health diagnoses and are experiencing severe behavioral disruptions that impact their functioning in family, school, and social environments. WISe is an evidence-based model with a collaborative, cross-system approach to creative problem-solving. Care Coordinators work with youth and their families to develop and implement individualized, cross-system care plans that work toward the family’s long-term goals. Services are provided where and when convenient for the family, sometimes out in the community and outside of regular business hours after 5pm. There will be a need to work occasional weekends as well. Care Coordinators must be skilled in managing team conflict, engaging a variety of personalities, facilitating meetings, and communicating with various audiences. Local travel and routine evening work are expected. This position is part of our WISe team in Yakima and will serve families throughout Yakima County.
Care Coordinator duties may include:
Maintains CANS certification throughout employment.
Conducts CANS screening and CANS full assessments.
Conducts and/or provides information to ensure full CANS assessments are completed every 90 days for assigned Wraparound with Intensive Services (WISe) children/youth on active caseload.
Demonstrates and utilizes knowledge of development in children, youth and young adults.
Demonstrates and utilizes knowledge and understanding of behavioral health problems in children, youth and young adults.
Demonstrates and utilizes knowledge of and ability to work with issues of confidentiality, consent and capacity.
Demonstrates the ability to collaborate and coordinate with family, parents, caregivers and community and natural supports.
Communicates effectively with children and youth of differing ages and developmental levels.
Qualifications:
Bachelor's degree in social work, psychology, or related field
Must be eligible for registration as Agency Affiliated Counselor
Must have valid driver's license and vehicle liability insurance
