The RN is responsible for Care Management for assigned consumers including: Teaming with internal and external providers, aggregating health data and ensuring medical record integrity, and providing direct nursing care that is emergent, urgent, and routine. The RN achieves key performance indicators: including service volumes, quality and timely documentation, beneficial outcomes as measured by health data and statistics, and teamwork. KEY RESPONSIBILITIES Conducts collaborative and systematic activities to ensure that all assigned consumers are engaged and assessed to identify, prioritize, and treat acute and chronic health risks and concerns. Champions preventative care and conducts frequent physical and mental health vital signs and screenings. Maintains medical record including quarterly and annual review, incorporation of pertinent medical records, historical log of vital signs and values, and other documentation as needed. Provides direct care including but not limited to: treatment planning, assessment, individual and family supports, health promotion, psychoeducation, and therapeutic groups. Supports medication management including administration via IM injection, dispensing in collaboration with Pharmacist, environmental structuring that supports adherence, teaching, and addressing barriers such as insurance changes and prior authorization. Documents all direct and indirect services in a timely manner, in the format specified by the assigned program, to a quality that confirms with professional standards and agency policies. Refers Consumers internally and externally for services as needed. Coordinates care with members of consumers' natural and professional networks by permission and as needed. Participates in discharge planning for based on readiness, eligibility, and engagement. Responds and manages physical and psychiatric emergencies, at the center and in the community. RN provides First Aid as necessary and appropriate. Completes OAS nursing/withdrawal assessments as assigned to determine level of substance abuse treatment needed. Participates in continuing education, certification, and training as required by agency, position, and licensure. Provides coverage for other RNs, as assigned. DESIRED KNOWLEDGE/SKILLS/ABILITIES: AA Degree required; BSN preferred Active RN license in the District of Columbia required CPR/BLS Certification required Ability to make sound and accurate judgement calls Ability to main confidentiality Demonstrates basic knowledge of mental health principles and demonstrated ability to operationalize and implement these principles in mental health practice. Demonstrates ability to understand complex clinical concepts (engagement, outreach, empathy, etc.) and interventions over time and to incorporate these into clinical practice under supervision of clinical manager.
