Overview:

Sentara Health Plans is hiring a Remote Care Coordinator for the Roanoke/ Lynchburg/ Danville area!

This position is remote work from home with travel to Nursing facilities required.

Hours of operation are M-F from 8:00am to 5:00 pm

Applicants must be in for the Roanoke/ Lynchburg/ Danville area.

The Care Coordinator performs a variety of casework duties and provides case management services to Members in Nursing Facilities, including patients, families, and designated caregivers and requires travel to assigned facilities. Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses

Education

Bachelors Degree (Required)

Bachelors Degree must be in Health & Human Services or one of the following related Fields:

-Behavioral Sciences
-Child Development
-Cognitive Sciences
-Community Mental Health Counseling (MH, Vocational, Pastoral, etc.)
-Counselor Education
-Early Childhood Development
-Educational Psychology

Experience

1 year of Long-Term Care and Health Plans experience (Required)

Nursing Home Experience (Preferred)

Case Management/Care Coordination experience (Preferred)

Experience with Community Health Resources (Preferred)

Experience initiating care plans and both subjectively and objectively conducting assessments (Preferred)
