JOB DESCRIPTION

Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance abuse, as well as an array of other related problems. Since 1969, FGC's comprehensive programming has educated and guided individuals toward their full potential as productive members within their own families and our society.

FGC's adult and adolescent programming provides a flexible continuum of care to best address the individual needs of each participant. Through integrated and comprehensive services, including continuous assessment, participants are able to access levels of care and specific services to create an individualized treatment approach. Further, FGC provides special and high-risk populations priority access to specialized programming.

Essential Duties and Responsibilities:

Complete FGC intake screening documents via face-to-face or telehealth to determine appropriate level of care.

Respond to bookings/emails for daily assessment assignments in a timely manner.

Administer the biopsychosocial assessments in EHR (SAMMS) with attention to clinical detail and justification from DSM.

Complete all intake paperwork including admission note within one business day of the intake.

Utilize the SOAP clinical note format for documentation.

Scan all admission documents into SAMMS within one business day of assessment.

Coordinate admission process with Retention Specialist to ensure successful completion of all steps by patient (i.e., toxicology, medical team/nursing).

Communicate to appropriate staff/referral sources as needed.

Work Location: 20 N 11th St, Springfield, IL 62703

Schedule: : Must work a minimum of 40 hours per week from 9:30am-6:00pm Wednesday through Friday, Saturday-Sunday 8:00am-4:30pm. Hours may vary based on program needs of the agency and flexibility is required.

Pay Rate: : $20.19 - $24.04 per hour (based on certification/experience)

Sign-on bonus available for certified external candidates.

Start Date: ASAP

Experience and Skills:

Education and/or Experience: High School Diploma required.

Certification, Licensure, Registrations: IAODAPCA Certified Alcohol and Other Drugs Counselor required, or must obtain within up to two years from date of hire.
