Work Status: Full-time | 40 Hours per Week Department: Social Services Job Summary Under the direction of the Manager of Social Services, this role will enhance the patient experience by assessing/addressing potential mental health issues that exist among the inpatient population, provide counseling, as needed, to inpatients and outpatients, coordinate the Palliative Program, offer education on a variety of mental health issues to staff and patients, and assist the Social Service team in meeting regulatory requirements and, most importantly, the social needs of our patients. Although this position will not be expected to participate in a regular call schedule, the probability is high that the individual will be asked to come to the hospital after hours to assist with a variety of patient care issues. For this reason, it is imperative that the individual in this position communicates and collaborates with Nursing and co-workers in the Social Services departments to ensure this type of coverage when he/she is available. Essential Functions Facilitate individual and group therapy session in person or telehealth using a secured, HIPAA compliant platform.Perform initial evaluations and develop a treatment plan. Monitor progress.Complete all documentation within the electronic health record (EHR). Documentation should be timely and thorough.Complete all requirements for coding and billing.Develop therapeutic processes.Provide culturally competent, and trauma informed care.Collaborate with the interdepartmental team as needed to provide treatment.Work within scope of license and make appropriate referrals when necessary if unable to meet the needs of the individual.Provide drug and alcohol evaluations. Participate in community coalitions related to behavioral health or at the discretion of the Director. Provide hospital staff, Medical Staff, patient, and community education regarding mental health as directed. o Identifying mental illness. Providing care to patients and/or family members with behavioral issues, mental illness or intellectual disabilities. Serve on Sexual Assault Response Team (SART) o Maintain needed skills/knowledge base through continuing education, reading, etc. Participate with team to develop policies/procedures, employee education, drills, etc. Enhance community awareness. Participate on the Obstetrical Team.Provide mental health/counseling services for Amplified Pain Syndrome (AMPS) program.Follow job description related to that hospice Social Work position.As a member of the Social Services team, lend counseling expertise to patients in a variety of patient care areas including, but not limited to: Pain therapy. Oncology. Cardiac Rehab. Emergency Department. As part of the Social Services team, assist staff with circumstances that may involve regulatory reporting and/or coordination and collaboration with other entities including, but not limited to: Work with law enforcement to effectuate emergency protective custody (EPC) as needed.Make reports to Child Protective Services (CPS) or Adult Protective Services (APS) as required by law or as need is indicated.Haven House. Facilitate group support sessions for a variety of patient types, i.e., oncology.Assist with assessment of inpatients identified by Nursing, Social Service or Medical Staff personnel as individuals who may have more complex mental and/or behavioral conditions that require a higher level of counseling services.Work with members of the Social Services team and others to meet requirements of the Medicare Conditions of Participation, particularly as they relate to mental illness, behavioral issues, and patients with intellectual disabilities.Learn and understand the role of providing social services and discharge planning to provide support to the social service team including home health and hospice.As a part of the Social Services team, collaborate with co-workers and the Quality Improvement team to improve patient satisfaction scores and enhance the overall patient experience.
