Psychiatric Care Coordinator

Position Summary

The Psychiatric Care Coordinator plays a key role in supporting an integrated behavioral health and primary care model within the outpatient clinic setting. This position assists clients in addressing both behavioral health and physical health needs through coordination of care, health screenings, basic clinical support, and linkage to community medical providers.

The Psychiatric Care Coordinator works closely with prescribers, clinicians, and external medical providers to improve health outcomes, reduce barriers to care, and support whole-person wellness.

Essential Responsibilities

  • Support the development and implementation of an integrated care model within behavioral health services
  • Assist and track clients in connecting with primary care providers, specialty medical providers, and community health resources
  • Collaborate with behavioral health clinicians and prescribers regarding client medical needs and treatment coordination to ensure continuity of care among providers
  • Complete health screenings and toxicologies during intake and ongoing treatment episodes, obtaining relevant medical history
  • Maintain accurate and timely documentation within the electronic health record
  • Provide basic psychoeducation related to wellness, preventative healthcare, medication adherence, nutrition, smoking cessation and chronic health conditions.

Qualifications

  • Medical Assistant certification and relevant healthcare experience
  • Strong organizational and communication skills
  • Ability to work collaboratively within a multidisciplinary team
  • Bilingual English/Spanish preferred
  • Proficiency with electronic health records and computer systems
  • Experience working in outpatient behavioral health or substance use treatment
