Your Role As the Transition Case Manager, you will focus on facilitating the transition of Colbert and Williams consent decree class members from nursing care facilities to community-based living. This position will be responsible for working closely with class members, government entities, landlords, and community providers to ensure class members have all necessary resources in place to successfully move into the community. Responsibilities
  • Develop relationships with community providers to provide linkage to resources necessary for a successful transition to community-based living.
  • Learn and maintain knowledge of housing resources; develop relationships with landlords and property managers to facilitate moving members into the community.
  • Assist members in obtaining benefits, including but not limited to, Medicaid and SSI/SSDI.
  • Assist members in obtaining needed items to transition to community-based living.
  • Advocate for member freedom of choice throughout the transition process.
  • Ensure delivery and/or coordination of all community services are in compliance with DHS Rule 132/140, CARF standards, agency mission, agency policy and procedure, program guidelines, and best practices.
  • Seek continuous learning about best practices in community-based services.
  • Collaborate with other teams and staff to enhance services.
  • Attend all agency mandated training within required timeframes.
Qualifications
  • Bachelor's degree in human services or related field required.
  • Knowledge and/or experience in mental health services.
  • Minimum of one year experience working with individuals with medical and/or psychiatric disorders and working knowledge of the recovery model preferred.
  • One year case management, linkage, outreach, and/or community support experience preferred.
  • Excellent communication skills, both oral and written.
  • Ability to work in a variety of environments and willingness to provide services in location most convenient to the individual served.
  • Valid drivers license and documentation of current auto insurance, with an acceptable motor vehicle report.
  • Proficient in the use of computers, software applications, and working knowledge of Microsoft Office Suite programs.
