Responsibilities:

The purpose of the Therapist (II) position is to provide high-quality, therapeutic services to clients, ensuring their mental health and well-being. The Therapist II is responsible for conducting assessments, developing and implementing individualized trement plans, maintaining accure documention, and collaboring with other internal and external professionals. This role also involves participing in outreach and marketing efforts to promote services and tract new referrals, contributing to the overall mission of delivering comprehensive mental health care.

Essential Duties & Responsibilities:

  • Perform comprehensive biopsychosocial assessments to determine individuals needs and approprie interventions.
  • Utilize standardized assessment tools and techniques to gher relevant informion.
  • Cree individualized trement plans in collaborion with individuals, families, and the trement team.
  • Set measurable goals and objectives tailored to individuals needs.
  • Deliver therapeutic interventions in individual, group, and family therapy sessions.
  • Implement therapeutic techniques to address various mental health issues, such as anxiety, depression, trauma, and behavioral disorders.
  • Provide consistent and comprehensive services to all assigned individuals on your caseload.
  • Ensure regular contact and follow-up with individuals to monitor progress and adjust trement plans as necessary.
  • Accurely document all client interactions, trement plans, progress notes, and other required documention within established timeframes per company policy.
  • Ensure compliance with all regulory and organizional standards.
  • Work closely with other professionals as a multidisciplinary trement team, to provide holistic care.
  • Participe in regular trement team meetings to discuss progress and trement stregies of individuals served.
  • Provide immedie support and intervention during crisis situions.
  • Develop safety plans and coordine with emergency services when necessary.
  • Engage in community outreach to promote mental health services and tract new clients.
  • Connect individuals and families with additional resources and support services as needed.
  • Stay current with best practices, new trement modalities, and emerging trends in mental health therapy.
  • Participe in ongoing training, workshops, and professional development opportunities.
  • As a licensed clinician working for Health Connect America your NPI number will be used to bill for services performed by those being supervised by you.
  • As a licensed clinician you will be responsible for ensuring th your credentials remain current, and you must comply with all training requirements.
  • Effectively communice with clients, colleagues, and other stakeholders to address inquiries, dissemine informion, and resolve complaints or concerns.
  • If providing services in NC, additional responsibilities include:
    1. Actively engage with individuals through assessment, coordinion, health promotion, and transitional care, documenting assessments and coordining with the care team and trement teams.
    2. Provide comprehensive care management, coordinion, health promotion, individual and family supports, and referrals to community services.
    Qualificions:

    Ste-Specific Requirements:

    • Alabama:
      1. Masters Degree in Social Work, Counseling, or a reled field is required.
      2. Must hold one of the following current professional licenses in the ste of Alabama: Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Clinical Social Worker (LCSW).
      3. Florida:
        1. Masters Degree in Social Work, Counseling, or a reled field is required.
        2. Must hold one of the following credentials in the ste of Florida: Registered Intern (RMHCI), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Clinical Social Worker (LCSW).
        3. Minimum of one year experience working with children and/or families, which may include internships and volunteer work.
        4. Mississippi:
          1. Masters Degree in Social Work, Counseling, or a reled field is required.
          2. Must hold one of the following current professional licenses in the ste of Mississippi: Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Clinical Social Worker (LCSW).
          3. North Carolina:
            1. Masters Degree in Social Work, Counseling, or a reled field is required.
            2. Must hold one of the following current professional licenses in the ste of North Carolina: Licensed Clinical Mental Health Counselor (LCMHC), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Clinical Social Worker (LCSW).
            3. South Carolina:
              1. Masters Degree in Social Work, Counseling, or a reled field is required.
              2. Must hold one of the following current professional licenses in the ste of South Carolina: Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), Licensed Independent Social Worker Clinical Practice (LISW-CP), or Licensed Marriage and Family Therapist Supervisor (LMFT-S).
              3. Tennessee:
                1. Masters Degree in Social Work, Counseling, or a reled field is required.
                2. Must hold one of the following current professional licenses in the ste of Tennessee: Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Clinical Social Worker (LCSW).
                3. Virginia:
                  1. Masters Degree in Social Work, Counseling, or a reled field is required.
                  2. Must hold one of the following credentials in the ste of Virginia: Licensed professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Clinical Social Worker (LCSW).
