Care Coordinator - BCM CHILDRENS CRISIS TREATMENT CENTER INC - 3.4 Philadelphia, PA Job Details Full-time 6 hours ago Qualifications Working with families Working with individuals with mental health conditions Full Job Description At Children's Crisis Treatment Center (CCTC), our staff is dedicated to addressing the impact of childhood traumatic events and other challenges. The Care Coordinator will support our Blended Case Management (BCM) program and follow through with treatment team recommendations in a faced paced environment and meet the needs of the children ages 2-14 and their families presenting with crisis and behavioral health issues in a trauma-informed manner. Summary of Essential Functions: Provide interagency assessment and understanding of child's medical, family, social, physical and educational history Develop interagency service planning based on the strengths of the child and family and assist in developing any activities which are necessary to help the child and family in the home and community Support network building for the child, utilizing a system's approach to include the child's family and community Meet with families within their communities once per month or more and develop aggressive and creative strategies to assist the child and family gain access to resources and required services identified in the treatment or service plan Identify, link, access, coordinate and monitor needed services to keep the child stable in home school and community Implement and follow through with psychological or psychiatric recommendations Monitor the effectiveness of services to ensure continuity of care in the mental health, behavioral and educational system Network with other child-service systems Act as a child and system advocate Provide services with cultural sensitivity Maintain open communication with team members and supervisor on caseload status Able to prioritize multiple tasks and meet paperwork deadlines Assume additional responsibilities as assigned A minimum of a Bachelor's degree in Social Work, Social Science or Bachelor of Arts Degree from an accredited educational institution. Ability to provide direct services to children and adolescents with serious emotional disturbances and their families. Experience in community families with multiple problems.
