Participates in interdisciplinary patient care conferences and team meetings; serves as consultant to other professionals on the healthcare team; participates in staff meetings, in-services, continuing education programs, research activities, CQI and performance improvement activities; teaches and mentors health professional students, regardless of discipline, on theory and practice of care delivery by an inter-professional team; serves as preceptor for Community Health Worker and Social Work students.
