Starting salary for this position: $39,852.80/annual ($19.16/hr.)

Position Summary & Responsibilities

Position Summary:

This position will serve as the Care Coordinator for participants involved in the Topeka Kansas Supportive Housing Program (KSHOP). Topeka KSHOP targets pre-release individuals with severe and persistent mental illness who also experience co-occurring substance abuse as well as a history of homelessness. The Care Coordinator will identify and screen participants for the program from correctional facilities throughout the state. The Care Coordinator will meet with identified participants within the facilities prior to release to develop a rapport, advocate for facility-based programming prior to the offender's release and connect the offender with community-based resources such as housing, mental health and substance abuse treatment prior to and upon release. Establishing partnerships with property managers/landlords, Astra Mental Health, community-based substance abuse providers and other community-based agencies/providers and connecting KSHOP participants with these agencies as a way to reduce recidivism. This position will work with providers, community groups, and KDOC staff (discharge planners, IPO, parole and reentry) to identify gaps and develop strategies for filling those gaps. In emergency situations only, this position may have to be available to problem solve with Astra Mental Health Staff, Parole and/or Landlords outside the traditional business hours for Valeo MOA participants within the community.

Job Responsibilities may include but are not limited to the following:

Serve as the liaison between KDOC and Astra Mental Health for the program to identify pre-release individuals with severe mental illness, drug and alcohol issues, and periods of homelessness who will be returning to Shawnee County and meet the KSHOP criteria. Meet with offenders prior to release to assess service needs either in person or via phone/Teams/Zoom. Work with facility staff to ensure SOAR/RADAC screenings (and any other screenings necessary) are completed prior to release. Establish a method for accessing Astra data and case information as needed for eligibility determination and case management while maintaining proper confidentiality.

Assess and identify housing needs for Topeka KSHOP participants, including transitional housing options. Collaborate with KDOC Community Reentry Specialists and Astra Housing Specialists to secure safe, stable, and permanent housing placements. Transitional housing is available to all releasing individuals who meet established eligibility criteria. Identify and screen eligible individuals pre-release for transitional housing consideration. Individuals preparing for release as potential candidates for transitional housing. Provide support and intervention as needed when housing placements are unsuccessful or when issues arise with participants or landlords. Ensure transitional housing units are fully furnished and prepared for occupancy prior to participant move-in by coordinating with community partners, KDOC, and Astra to identify needs and secure necessary resources.

Immediately upon release, connect Topeka KSHOP participants to community based resources based on their identified needs. This may include but not be limited to mental health, substance abuse treatment, etc. Remove barriers such as transportation that may limit service provision. Provide on-going one-on-one case management for Topeka KSHOP participants who are not receiving case management services through Astra either because the service has not yet been started or because they do not qualify for Medicaid. Responsibilities may include securing permanent housing, connecting to community based services, conducting house visits, etc. Record interactions with offenders and collateral contacts in the Athena data base.

Facilitate multi-disciplinary meetings with community partners. This will include staffing program referrals with Astra Mental Health to ensure the participants meets the necessary criteria, staffing cases as needed when challenges or concerns arise, and providing programmatic updates to agency administrators on a quarterly basis.

Monitor and track identified monthly data information with the participant and complete entry of said data in collection tool.

Qualifications

Minimum Qualifications:

Six months of experience in planning, implementing and monitoring activities relevant to the agency's programs. Education may be substituted for experience as determined relevant by the agency.

Substitution: A bachelor's degree with major coursework in criminal justice the social sciences, the behavioral sciences, law, or public administration.

Preferred Qualifications:

Licensed Masters Social Worker.

One to two years related experience and/or training.

Experience in providing case management to persons with severe mental illness.

Training and/ or experience in cognitive programming.

Demonstrated skill of problem-solving, multi-tasking and handling a substantial work load.

Demonstrated ability to network and connect system pieces/ players on challenging access issues and sustaining collaborative partnerships.

Strong organizational, interpersonal and engaging skills.

Strong oral and written communication skills.

Working knowledge of MS Office Suite.

Strong willingness to learn new skills and participate in ongoing learning.

Ability to read, analyze and interpret governmental regulations.

Ability to effectively present information, respond to questions and complaints from widely varied constituencies.

Post-Offer, Pre-employment Requirements: Must possess and maintain a valid driver's license. Must pass a criminal history records check.
