The Prevention Specialist - Case Manager will assist in promoting prevention training and resources to regional school districts, community agencies, businesses and individuals. Provides direct case management services to individuals with severe and persistent mental illness and is responsible for developing, coordinating, and monitoring all programming activities for the C.A.R.E. program.

Job Knowledge and Comprehension:

Provides resource materials to regional school districts, community agencies, organizations, businesses and individuals in northeastern South Dakota.

Organizes, conducts and/or attends regional trainings and education to schools and community groups within the region.

Assist in developing prevention resources and preparing materials for training, presentations, and meetings.

Provides specialized treatment, assistance, education, rehabilitation, and support services as part of a comprehensive team to C.A.R.E. team clients.

Completes comprehensive assessments and evaluations necessary for establishing client eligibility for C.A.R.E. services.

Assists C.A.R.E. program clients in developing work skills and attitudes, and in finding, securing and maintaining community-based employment.

Performs other duties as assigned.

Physical Requirements:

Required to sit for long periods of time. Infrequent light physical effort required lifting up to 10 lbs. occasionally and 25 lbs. rarely. The employee frequently is required to sit and reach with hands and arms. Employee may be required to work extended periods of time at a computer monitor. Must be able to drive vehicle frequently.

Bachelor Degree in Public Health Education, Sociology, Psychology, Social Work, Family & Consumer Sciences, Community Development or closely related field.

The Board of Directors of the Human Service Agency shall have the prerogative to hire staff whose qualifications may vary from the above. This position requires access to Electronic Personal Health Information (EPHI). Current HSA Policies and Procedures apply. It is the responsibility of the employee to read, understand, and comply with HSA policies and procedures that apply to protected information and security. Services provided by HSA will be welcoming, accessible, integrated, and responsive to the complex needs of people with co-occurring disorders/conditions.
