I. SCOPE OF ROLE:The Wellness Specialist provides services and consultation as a member of a mobile multi-disciplinarytreatment team ( ACT team ).
II. ESSENTIAL DUTIES & RESPONSIBILITIES: Participates in daily treatment review meetings Provides support to all specialist on the team, addressing overall consumer wellness. Serves as a back up to the substance abuse specialist when absent Provides support around all services with regards to wellness, medication management, health,appointments, and other individualized goals Responsibility for documentation of services provided particular in the development of theservices plans Responsible for ensuring revisions to the plan as the participants needs change Advocating for the rights and preferences of the participants needs change The Wellness Specialist develops wellness interventions, activities and groups, drawing from theWellness Self-Management (WSM) curriculum and other resources; Educates participants aboutself-help techniques, processes and community options; Teach coping strategies based onpersonal experience Assist participants with acquiring symptom management skills; Assist in identifying and clarifyingrehabilitation and recovery goals Assist in the development of community support systems and networks Consistently practices according to the principles of recovery-oriented and trauma-informedservice delivery Comprehensive ACT service activities include: Assessing and providing services to participants toaddress health and wellness, housing, income support, education, vocational training,employment and social supports; Complete initial comprehensive recovery-oriented service planand 6-month plan review incorporating participant and relevant others feedback Provide ACT treatment services including: service planning and coordination; problem solving;support with obtaining housing; developing social connections; strengthening family and otherrelationships; developing independent living skills and obtaining necessary resources; accessingand accessing education and training; employment supports (job search, placement and support);entitlement and financial management; empowerment and self-help; wellness self-managementwith a focus on the development of coping skills; support with medications; and weekly groups Use motivational interviewing, recovery and trauma-informed approaches when deliveringservices; Complete crisis intervention and relapse prevention plans; Collaborative with
participants, families and natural supports, shelter and community partners to promoteattainment of treatment and recovery goals Coordinate discharge in collaboration with inpatient and ER staff when participants haveinvolvement; Complete thorough, timely and accurate documentation and adequateauthorization records consistent with billing, regulatory, policy and contractual requirements Complete expected minimum monthly treatment contacts of which 80% occur in the community;Provide 24-hour crisis intervention on-call services on rotating basis
QualificationsREQUIREMENTSREQUIRED EDUCATION AND EXPERIENCE Masters degree in social work, psychology, rehabilitation counseling, or a related field Must have excellent verbal, written and interpersonal communication skillsPREFERRED QUALIFICATIONS & SKILLS One year of experience working with adults with severe mental illness Bilingual in any language a plus not required Basic computer skills (e.g. MS Office suite and Windows)IV. PROGRAM-SPECIFIC DETAILS:ACT is an intensive, community-based multidisciplinary treatment team that provides person-centeredcare to a shared caseload of adults whose needs are not able to be met through traditional outpatientservices.
