SUMMARY

The Registered Nurse works as part of a Forensic Assertive Community Team th provides care and trement for clients th are experiencing significant mental health/psychiric symptoms, who are demonstring significant functional impairments, and who may have co-occurring chemical dependency and/or personality disorder. Primary duties include health/medicion assessment, crisis assessment and management, planning, individual counseling, skills teaching, psychiric/medical case management, medicion monitoring/administrion, staff training, care coordinion with outside providers, and on-call.*FACT is an adaption of the assertive community trement model designed to serve people with a serious mental illness who are involved in the criminal justice system.

ESSENTIAL JOB FUNCTIONS

To perform this job successfully, an individual must be able to sisfactorily perform each essential function listed below.

  • Work in full partnership with the client in his/her recovery process.
  • Promote for the care, welfare, trement, and safety of the clients as individuals who may have significant mental health/psychiric symptoms and may have co-occurring chemical dependency and/or personality disorders.
  • Conduct health/medicion assessment, crisis assessment and management, planning, individual counseling, skills teaching, psychiric/medical case management, medicion monitoring/administrion, staff training, care coordinion with outside providers, and on-call.
  • Provide crisis and behavioral management techniques, goal directed counseling, and medical/psychiric case management.
  • Work with a forensic assertive community team with an adaption of the assertive community trement model designed to serve people with a serious mental illness who are involved in the criminal justice system. Work collaborively with the mental health professionals and the criminal justice agencies.
  • Deliver nursing care, monitors trement, and administers medicion to individuals with varying levels of mental health disorders and/or intellectual and developmental disabilities, in accordance with established nursing practices and guidelines.
  • Coordine communicions and actions of various disciplines as it reles to health and safety support for the pient.
  • Complete daily, monthly, quarterly and annual nursing documents as/ if required and ensure documention of individuals health is in nurses notes.

Minimum Knowledge and Skills required for the Job

The requirements listed below are representive of the knowledge, skill, and/or abilities required to perform the job.

Educion and Experience:

  • Associe arts degree in nursing and has least 6,000 hours of supervised experience in the delivery of services to persons with mental illness; or bachelor's degree in nursing and has least 2,000 hours of supervised experience in the delivery of services to persons with mental illness; or holds a gradue degree in nursing and has least 2,000 post BA hours of supervised experience in the delivery of services to persons with mental illness.

Certifices, Licenses, and Registrions:

  • All staff are supported in gaining certificion as a Certified Psychiric Rehabilition Practitioner.

Other Skills and Abilities:

  • Ability to problem-solve.
  • Communice skills, orally, and in writing.
  • Relionship management skills.
  • Ability to work alone.

Other Requirements:

  • Travel as needed.

Physical Requirements:

  • Medium Work: Exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
