Job Description:
The Care Coordinator provides comprehensive care coordination services to youth, adults, and families to promote access to integrated healthcare, behavioral health, and community-based services. Acting as a trusted liaison between patients, families, providers, schools, community agencies, and other stakeholders, the Care Coordinator helps patients navigate complex systems of care while advocating for their individual needs and goals.
Using a strengths-based, trauma-informed, and culturally responsive approach, the Care Coordinator supports patient engagement, service planning, resource linkage, care transitions, and multidisciplinary collaboration. This role contributes to Valle del Sol's mission by promoting whole-person wellness, reducing barriers to care, and improving health outcomes through coordinated and integrated service delivery.
Job Qualifications:
Education
Required:
- High School Diploma or GED.
Preferred:
- Associate's or Bachelor's degree in Social Work, Psychology, Counseling, Human Services, Public Health, Healthcare Administration, or a related field.
Experience
Required:
- Experience working with individuals, families, or communities in behavioral health, healthcare, social services, education, case management, care coordination, or a related field.
- Typical experience levels:
- Care Coordinator I: 02 years of relevant experience
- Care Coordinator II: 25 years of relevant experience
- Care Coordinator III: 5+ years of relevant experience
Preferred:
- Experience coordinating services for individuals with behavioral health, medical, or social service needs.
- Experience working within integrated healthcare or community-based settings.
- Experience facilitating team meetings, service planning, or patient advocacy activities.
Licensure/Certification
Required:
- Must meet qualifications for and obtain Behavioral Health Technician (BHT) credentialing within 90 days of hire and maintain credentialing requirements throughout employment.
- Must meet the licensure, certification, or credentialing requirements applicable to the position in accordance with Arizona law and applicable regulatory authorities.
- Must practice within the scope of applicable credentials consistent with organizational policies and the needs of the populations served.
- BLS certification, or ability to obtain within 5 days of start date.
- Arizona Level I Fingerprint Clearance Card, or ability to obtain within 5 days of start date.
- CPI certification within 90 days of hire.
Preferred:
- Community Health Worker (CHW) certification.
- Case Management Certification (CCM) or other care coordination-related certification.
Other Requirements
- Must be at least 18 years of age.
- Must be authorized to work in the United States.
- Must be able to pass pre- and continued employment screenings successfully.
- Must maintain all required trainings, certifications, immunizations, and credentialing requirements in accordance with Valle del Sol policies.
- Ability to travel between Valle del Sol locations and community sites as required.
