Clinical Case Manager

JOB REQUIREMENTS: Manitowoc County Human Services Department is seeking

a Clinical Case Manager. This position provides and facilitate services

for individuals with a mental illness with the goal of avoiding

institutionalization and enabling the individuals to live in the

community in the most productive and least restrictive manner possible.

This position: Provides group and individual counseling to mentally ill

clients. Provides case management, direct services and coordination of

services with other providers. Assists clients with improving daily

living skills. Provides crisis intervention services to individuals

experiencing mental health or substance abuse crisis situations.
