DUTIES AND RESPONSIBILITIES**
1. **Case Management:**
2. - Coordinate and oversee client caseloads, utilizing effective case management techniques.
- Maintain client medical histories and manage referrals, scheduling, and treatment adherence.
3. **Client Counseling and Support:**
4. - Counsel clients individually or in groups to address issues like substance abuse, mental or physical illness, and social challenges.
- Assist clients in adhering to treatment plans and provide support through referrals and appointment arrangements.
5. **Collaboration and Coordination:**
6. - Collaborate with healthcare professionals (counselors, physicians, nurses) to plan and coordinate treatment based on social work experience and patient needs.
- Ensure effective communication and coordination with primary care physicians.
7. **Client Assessment and Monitoring:**
8. - Conduct client interviews, assessments, and evaluations to monitor progress toward treatment goals.
- Modify treatment plans as needed based on changes in client status.
9. **Education and Community Engagement:**
10. - Educate clients and community members about mental and physical health issues, medication, and available resources.
- Plan and conduct community programs to address social problems and improve health services.
11. **Supervision and Leadership:**
12. - Supervise and direct other clinicians providing services to clients.
- Function in a leadership capacity as designated by the Program Director or Designee.
13. **Confidentiality and Compliance:**
14. - Maintain client confidentiality at all times, adhering to ethical standards and reporting any abuse or suspected abuse.
- Enter, maintain, and monitor confidential client information (PHI) in compliance with regulations.
15. **Professional Development:**
16. - Increase social work knowledge through literature review, research, and attending seminars or workshops.
**Additional Responsibilities:**
- Conduct routine clinical staff audits and ensure accurate documentation of client progress and referrals.
- Utilize therapeutic interactions, such as CBT and DBT, to sustain ethical therapeutic relationships.
- Work independently under pressure and meet deadlines.
- Develop or advise on social policy and assist in community development.
