The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

GENERAL PURPOSE:
The Mental Health Therapist assigned to the Assertive Community team is responsible for providing psychosocial assessment and treatment to individuals ages 18 and older who are severely and persistently mentally ill. This is provided through the Mental Health Therapists participation on an ACT outreach team designed to provide comprehensive community based psychiatric care. The Mental Health Therapist functions a general therapist for assigned patients and provides direct clinical service including individual and family therapy, group therapy, crisis intervention, and community
outreach.

ESSENTIAL FUNCTIONS

  • Provides direct clinical services to assigned patients, which may involve individual, family, and/or group therapy, crisis intervention, and diagnostic evaluations.
  • Participates as a member of an interdisciplinary team involving consultation, treatment planning, and periodic care review.
  • Collaborates with family, social supports, community agencies (past and present) to ensure appropriate and effective care. Coordinates referrals and facilitates linkage to other appropriate agencies/supports.
  • Documents interventions, both direct and indirect, in the patients medical record in compliance with the Division, Department, and Hospital policy.
  • Assists clients in maintaining themselves in the community, including setting up financial budgets, applying for entitlements, learning skills of daily living, and other case management tasks as indicated.
  • Transports clients as needed to appointments and to meet treatment plan goals.
  • Ensures that Hospital and Department and policies and procedures are implemented with regard to professional practice, patient care, and disposition training.
  • Subscribes to the Departments goal of creating a culturally competent environment by treating patients/families, trainees, research subjects and co-workers in a sensitive and caring manner with appropriate attention to cultural differences. Participates in available cultural competence events and trainings.
  • Participates in a client on-call phone rotation with other ACT team members approximately every 3-10 weeks, as needed and according to staffing.

QUALIFICATIONS

Required:

  • Master's degree from a Counsel for Accreditation of Counseling and Related Education Programs (CACREP) accredited program with major course work in mental health counseling required
  • Working with the SPMI (severe and persistently mentally ill) patient population required, preferably in a community-based setting.
  • Or equivalent combination of education and experience
  • NYS licensure as a mental health practitioner (e.g., Mental Health Counselor, Creative Arts Therapist) or limited permit eligible upon hire
  • If licensure has not been obtained upon employment, the individual has 36 months to obtain the NYS license
