Summary:

The Case Manager (CASE MANAGER-FSP) provides a community-based service to the caregiver of a child/youth that has mental illness, substance use disorder or co-occurring diagnoses. Case Manager-CASE MANAGER-FSP provides structured, one-to-one, person-centered, strength-based interventions that promote self-determination, self-advocacy and focus on recovery and resiliency. The service provided to the caregiver must be directed exclusively toward the benefit of the child/youth. A CASE MANAGER-FSP is expected to improve the outcomes for child/youth that are involved in multiple systems and increase the child/youth and the family’s capacity to manage their own services and support while promoting recovery and healthy relationships. The CASE MANAGER-FSP service confirms the belief that family voice and choice are essential in care and in producing positive outcomes.

In a mobile crisis response, the CASE MANAGER-FSP connects to the family during or shortly after the initial crisis response and assessment. The position provides mobile crisis support for individuals and families who are utilizing the Mobile Crisis Response Team. A CASE MANAGER-FSP is a person who can articulate lessons learned from their own lived experience and has specialized training to assist and empower families raising young people who experience emotional, developmental, behavioral, substance use, or mental health concerns. The Family Partner works within the System of Care (SOC) values and principles.

Essential Duties and Responsibilities :

  • Provides Family Support Partner duties, referring and linking to needed services, monitoring/follow up with client and referrals, provide education for health promotion.
  • Participates in interdisciplinary treatment planning, consultation activities and ensures all involved parties are aware of the plan of care.
  • Provides crisis intervention to all participants of MORES and involves crisis services when needed.
  • All other duties as assigned by supervisor.
  • The position responsibilities of the MORES Mobile Crisis Family Support Partner include, but are not limited to, the following:
  • Knowledge of family driven and youth-guided care, family supports, services coordination, and connections, principles of human behavior and interpersonal relationships and general record documentation.
  • Skills in
    • Engagement of youth and family/caregiver
    • Negotiating and de-escalation with clients and family/caregivers
    • Identifying services within the established service system and uncovering natural supports
    • Motivational interviewing behavior change strategies.
  • Ability to:
    • Partner with crisis team to assess strengths, identify services and develop plan of care.
    • Report and document findings
    • Demonstrate a positive regard for clients and families.
    • Be persistent and remain objective.
    • Work independently, performing duties under general supervision.
    • Communicate effectively.
    • Develop rapport and communicate with persons from diverse cultural backgrounds.

Units Billed Minimum Requirement:

The CASE MANAGER-FSP will be assigned contacts to ensure the team meets the following requirements.

CASE MANAGER-FSP contacts for members with behavioral health needs:

High Acuity: Face-to-face contact every 24-48 hours with clinician in their community. Typically need immediate stabilization and lasts 1-2 days.

Moderate Acuity: Face-to-face contact every 48-72 hours and phone contact 3-4 days per week.

Low Acuity: Face-to-face contact a minimum of 1 contact per week and phone contact a minimum of 2 contacts per week.

Education and/or Experience: Below are the requirement that must be met to qualify for this position.

  • The Family Partner must be a parent/caregiver of a child who has received services and therefore has firsthand experience within the family system.
  • The Family Partner must be nationally certified or approaching national certification in becoming a Certified Family Peer specialist
  • Must have either :
    • Bachelor’s degree in a human services field from an accredited university and one year of experience working with the target population;
    • An associate’s degree in a human services field from an accredited school and two years of experience working with children/adolescents/transition-age youth;
    • A high school diploma or GED and a minimum of four years of experience working with children, adolescents or transition age youth.
