The Case Manager within the CRU (Crisis Residential Services) coordinates all services necessary to maximize consumer recovery and independence for individuals with mental illness including implementing strategies for obtaining services and supports that are goal-oriented and individualized. The CRU provides short-term alternative to inpatient psychiatric hospitalization for individuals experiencing an acute psychiatric crisis.

How you will change lives…
Case Management
- Ensure consumer involvement in the Person Centered Planning Process (PCP).
- Facilitate the implementation of Individual Plan of Service.
- Perform comprehensive, ongoing psychosocial assessments.
- Participate in legal proceedings including petitioning and testifying in clinical emergencies.
- Provide community based services.
- Facilitate the coordination of consumer intakes, discharges, and transfers.
- Communicate with all service providers in Treatment Plan.
- Facilitate hospitalizations, attend hospital meetings and coordinate with hospital social worker.
- Coordinate and monitor activities such as a job, school, volunteerism, and family caretaking.
- Provide referral to appropriate community resources.

We are Hopeful you have…
- Bachelor’s degree in a human services field from an accredited institution required.
- LLBSW, LBSW, LLMSW, LMSW preferred.
- Prior case management experience in a mental health or rehabilitation setting preferred.
- Valid Michigan Driver’s License required.
