Summary:Provide nursing care to clients with serious and persistent mental illness and participates in rehabilitation and treatment programs
Duties:Provide direct and supportive consumer services for the Assertive Community Treatment (ACT) team including telephone triage, obtaining vital signs, refilling prescriptions electronically and implementing prescribers orders, coordinating care with other providers, initiating prior authorizations for medications, assisting in referrals to hospital or crisis stabilization facilities as needed, electronic documentation, and special projects as needed.
Assist the assigned psychiatrist and/or Advanced Practice Registered Nurse in the process of evaluation, treatment planning and ongoing treatment of patients, including patient education.
Complete all initial intake paperwork, carry a caseload, compile and complete all necessary paperwork as directed by program standards.
Provide direct services including crisis evaluation and triage, which includes assessment of patient's response to medications and side effects, along with patient education.
Make judgments about urgency and the need for psychiatric evaluation. Assess patients to detect behavior patterns and reports observations to medical staff as indicated.
Provide care coordination (for both mental and physical health care), including telephone liaison with patients, families, staff, and others.
Minimum Qualifications:A.A. or B.A./B.S. Degree in Nursing.
Licensed RN in Minnesota.
Knowledge of Community Psychiatric systems and resources.
Reliable Transportation
Desired Qualifications:One (1) year of experience as psychiatric nurse, with preference for previous nursing experience in an Assertive Community Treatment Program. OR
o Previous work experience in Community Mental Health Clinic, or as triage nurse, an outpatient mental health clinic nurse, or as a nurse on the inpatient unit.
Knowledge and working experience with chemical dependency issues
