Under the direction and supervision of manager and supervisors from the Comprehensive Community Services (CCS), work involves providing comprehensive, person-centered, team based, treatment to adults and transitional age youth living with mental illness, substance abuse or co-occurring disorders.

Work includes assessment of mental health and substance abuse, as well as individual, family and group psychotherapy to address both mental health and substance abuse needs. Additional services may include psychosocial rehabilitation, skill development, and other services on the CCS service array according to individualized recovery plans.

In conjunction with service provision, responsibilities may include the development and maintenance of client case records, benefits, plans and reports in accordance with DHS 36 and other relevant program codes, Social Security and Medicaid/Medicare requirements. This position requires collaboration with a treatment team, other HSD employees, families, and collateral agencies.

Supports and promotes the Mission, Vision and Guiding Principles of the Human Services Department. Performs in a manner consistent with the HSD Professional Standards and Commitment to My Co-Workers. .

60 %- Provide assessment and psychosocial rehabilitation services as a CCS psychotherapist under DHS 36.10(2)(e)1 that are person-centered, recovery oriented, trauma informed, and community based in accordance with DHS 36 and other relevant program codes. Provide interventions including; assessment, diagnostic evaluation, psychotherapy, psycho-education, skill development, recovery support services, service planning, and service facilitation. Provide services on an individual and group level including supportive services and education to families/caregivers.
  • Assessment and diagnosis under DHS 36 which includes the use of American Society of Addiction Medicine (ASAM) criteria and other standardized assessment tools.
  • Provide on-going consultation and treatment to include psychotherapy, psycho-education, skill development, and recovery support services when written into recovery plans. Treatment can be provided as individual, family and/or group services as appropriate.
  • Coordinate and co-facilitate groups as needed.
  • Advocate and provide services to support individuals in the least restrictive living situation possible and work to prevent out-of-home placements for consumers with complex needs. Develop/contribute to Behavior Response Plans. Attend and testify at court proceedings as needed.
  • Act as a team member on the consumer's treatment team contributing to the functioning of the treatment team, supporting and covering for other team members as necessary. Respond to client emergencies as requested to support the individual, families, and the client's team.
  • Regularly assess client mental status, areas of functioning, suicide and homicide risk, safety concerns, achievement towards stated goals, and symptom patterns and communicate this assessment to the team. Coordinate with internal and external providers, including Crisis and medication management.
  • On an as needed basis, provide other interventions for teams which you are actively on the service plan, for example service facilitation.

35%- Complete administrative tasks in accordance with DHS 36 and other relevant program codes, Social Security and Medicaid/Medicare requirements.
  • Using the appropriate format, accurately document all client contacts in the designated database system, providing as much detail in each client note as required to both communicate important clinical information and provide documented justification for billing for services.
  • Document 100% of time in Avatar, complete time sheets and other documentation as required by agency policy.
  • Complete/contribute to clinically relevant individualized treatment plans as required for each DHS 36. Complete/contribute to written discharge summaries. Complete other reporting requirements as assigned.
  • Attend in-services, trainings, division/department meetings, committee meetings and trainings as requested or approved by your supervisor.
  • Attend and participate in regular supervision and/or consultation to support specific treatment modalities, such as substance use treatment and/or DBT.

5% Perform other duties as assigned .- Knowledge of assessment and treatment of adults and transition age youth with complex mental illness and/or substance abuse disorders including co-occurring disorders.
- Knowledge of assessment techniques including ASAM criteria, the ability to make sound assessments and form solid clinical plans based on the assessment information.
- Knowledge of the principals and practices of psychotherapy. The knowledge of best practices and the ability to implement them.
- Ability to embrace and nurture a person centered, trauma informed, recovery oriented framework and culture for service delivery.
- Extensive knowledge of therapeutic and psychosocial rehabilitation interventions for persons with complex needs.
- Knowledge of the laws, regulations and practices pertaining to DHS 36, 48, and 51.
- Knowledge of psychotropic medications, their effect, side effects and abuse potential.
- Knowledge of community resources and how to effectively utilize them.
- Ability to plan and organize work effectively.
- Ability and skill to maintain accurate current records and prepare clear and concise reports.
- Ability to establish and maintain effective working relationships with agency staff, community members, families, and collateral agencies.
- Knowledge of various cultures and diverse populations and ways in which cultural values impact individuals, families, and the community.
-Ability to manage one's time and emotions in an environment of rapidly changing priorities.
-Ability and comfort in assessing risk and safety planning.
-Ability to accept feedback and apply feedback given by supervisor or consultant. -Masters degree or higher from an accredited college or university with a major in psychology, social work, counseling or related field
-Wisconsin Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Clinical Social Worker (LCSW) license required or ability to obtain license within three years.
-2 years of experience with treating individuals/families experiencing severe and persistent mental illness, trauma, and/or intellectual disabilities.
-Must possess and maintain a current unrestricted (not due to corrective lenses) driver's license and auto insurance pursuant to the Rock County Administrative Policy and Procedure Policy 5.02 and 5.27.
-Must complete 20-40 hours of training specific to CCS and DHS 36 within three months of hire date. Physical elements:
- Ability to stand, walk and sit. Physical movement occurs a majority of the day.
- Ability to enter client homes without assistance, including the ability to navigate stairs.
- Inside and outside work with exposure to weather conditions.
- Potential for contact with unpredictable and aggressive persons.
- Must be willing to transport consumers in your personal vehicle and be exposed to challenging and unpredictable environments.
- Participate in disaster evacuations and fire drills when circumstances warrant. Maintain safety procedures.
- Lifting and carrying up to 25 lbs frequently.

Equipment use:
- Ability to use a computer and keyboard.
-Must become efficient at using a computerized documentation within 2 months of hire including the use of Microsoft Office products, an electronic medical record system, and other web based systems.
-Ability to use a copy and fax machine as well as a cellphone.

Working conditions:
- Able to work in multiple locations including the office, client's home, and a variety of community locations.
- Available to work a flexible schedule including working evenings when needed to meet client needs.
- Ability to flex time when needed to coordinate consumer emergencies, care and recovery plan services.
- Potential exposure to physically or verbally abusive persons.
-Potential exposure to unsanitary homes and high stress environments.
- Potential for interactions with animals including but not limited to household pets and horses.
